Colorado Department of Revenue – Withholding Tax

Agency Information

Add to existing company setup

Submit a request to your service team to add CO (withholding) to your company setup.  Please include the below with your request:

  • Withholding – Deposit Frequency and Account number.  If you’re unsure of your deposit frequency or account number, you can call the agency at 303-238-7378.

Colorado Family and Medical Leave Insurance Program (FAMLI)

My FAMLI+ Employer is the online services portal Colorado businesses, third party administrators (TPA’s) and local government employers use to manage their FAMLI accounts. My FAMLI+ Employer | Family and Medical Leave Insurance

Register a new company

You can apply for both taxes online at https://apps.colorado.gov/apps/jboss/cbe/index.xhtml.  This will get you registered with both the Department of Revenue and the Department of Labor. Once you have registered online, you will immediately receive your account information followed up by a letter in the mail.

Power of Attorney

Power of Attorney is required, please obtain the prefilled form from your client service representative.

Colorado Department of Labor and Employment – Unemployment tax

Add to existing company setup

Submit a request to your service team to add COSUI (unemployment) to your company setup.  Please include the below with your request:

  • Unemployment – Tax rate and Account number.  This can be found on a Tax Rate Notice or the Quarterly contribution and wage report.  If you’re unsure of your tax rate or account number, you can call the agency at 800-480-8299.

Power of Attorney – POA is required, please obtain the prefilled form from your client service representative.
